Upload Application form Part 1 (PDF document only)

  • Click here to download Part 1 of application

  • The application includes the 'declaration form' to be attested by the Head of Institution

  • The duly filled, signed & attested application form is to be scanned and saved as PDF document to upload

Upload Application form Part 2 (PDF document only)

  • Click here to download Part 2 of application

  • The duly filled word document is to be saved as PDF document and uploaded

Step5

Upload Annexures (PDF or JPEG document) if applicable

  • a. Case record form

  • b. Questionnaires

  • c. Consent forms (in English and local language)

  • d. CV of co-investigators (if a multicentric study)

  • e. Consent from HOD (if applicable for co-investigator)

  • f. Ethics committee approval (if obtained)

  • g. CTRI registration submission acknowledgement (if obtained)

Step6

Submission & Acknowledgement

  • Once you Submit, an email confirmation will be sent to Principal Investigator with an application id for reference
